4.  HOW TO USE THE ORDER FORM Click on the red "Go to Order Form" button below and a new browser window will open for you with the Order Form page.  This will allow you to check back to this page for instructions and examples.  This IS NOT an online submission order form.  The Order Form has been set up to allow you to type your information onto the form online before printing it out.  The following hints will assist you in completing and printing out the form: type your information in each of the boxes use the TAB key to move through to the next box when you are ready to print the Order Form, for the page to print on one page, make sure the margins (left and right and top and bottom) in the Page Set Up are set to 4 mm if not sure, use your Print Preview you now have a typed order form to send by mail

We have also made available Order Form Downloads.  Once the form is completed, and you have saved the information in either a Word or PDF Document, you may send to us by email or by regular mail.
